Mr. Howell was admitted into The Phoenix Nursing and Rehabilitation Center on July 10th, 2018, from the Howard University hospital. He was admitted for rehabilitation of his broken legs, which he suffered while he was in Washington D.C. for a business meeting. He was on his way to the meeting when he approached a large amount of steps while carrying bags and luggage; unfortunately, he was in a rush and accidently fell down the stairs.

Mr. Howells’ perception of the community was very positive from the very beginning. He was welcomed and greeted by staff, and he was extremely happy about how friendly the staff were. He was going through a lot of stress due to his injury, so the friendly staff were more than welcomed! Mr. Howell was eager to start the process so he could get back to work. He was also impressed on how clean the community was and liked how the community was under renovation. The community was also very convenient for him since he lives just a few blocks away.

Mr. Howell met some of the staff from directors, nurses, housekeepers, social workers, and the concierges. He was very impressed by the concierge, Rafael Saldivia; they developed a great bond. Within the first 48 hours, Mr. Howell was already adjusted to his new room and was in the process of starting his rehabilitation. The goals of his care plan were to proceed with physical therapy on both legs, with different exercises and a lot of walking, so that he would be able to get the strength back into his legs.

During a few weeks into his stay, he was beginning to see some strong improvement in his legs, he felt like the physical therapy was starting to truly help with his recovery. Mr. Howell had positive experiences at the community. He attended recreational activities such as concerts, painting events, and also attended a Yankee game that the Phoenix community went to as a family!

The concierge devoted his time in checking up on Mr. Howell. They played board games such as chess, checkers, and they would also do a lot of talking about each other’s personal life. This relationship meant a lot to both Mr. Howell and our concierge. Mr. Howell was an entrepreneur who had his own business, which he was very passionate about.

After staying at The Phoenix for a few months, his time at The Phoenix was coming to an successful end. Mr. Howell was able to obtain in-home healthcare with a home attendant, which will be at his place of residence. His discharge date was set on October 15th, 2018.
It was a pleasure having Mr. Howell at our community. He was always happy, respectful, positive, and always in overall great spirits. We wish him nothing but success and great health!

Mr. Victor Novoa was admitted into The Phoenix on August 31, 2018, from the Brooklyn Hospital Center for rehabilitation of his leg, he also suffered from a mild stroke.

Mr. Novoa’s perception of our community was very positive. He was very pleased with how clean the building was and how friendly the staff were to him. When he arrived to the community, he met the wonderful staff. When he met the concierge, he felt very welcomed because the concierge went with him to his room and greeted him with welcome gifts. He also met the social workers who were very detailed, polite, and knowledgeable about what he needed to have done while staying at The Phoenix.

For the first 48 hours of his stay at The Phoenix, he spent a lot of time with his wife who came to visit him every day, he also began his rehabilitation process which was going very well. The care plan was to help him get the strength to begin walking again with the walker.

Mr. Novoa had great experiences at The Phoenix. He began to participate in different activities that the recreation staff were hosting such as, board games, tea parties, and concerts. Mr. Novoa was enjoying a successful rehabilitation process while working with different walking exercises. Mr. Novoa would also spend a great deal of time with the patient concierge, discussing personal information about his life, as he wanted someone he could open up to and interact with.

Mr. Novoa’s discharge date was September 24th. He was discharged back into his place of residence where he would continue receiving intensive, loving care from his family and friends. He was now able to walk with a walker so that he could continue living his life to the fullest. It was a pleasure having Mr. Novoa here at our community. He was always in great spirits, very positive, interacted well, and was a pleasure for other residents and staff. We as a community wish him nothing but great health and success!

Ms. Clyde was admitted to The Phoenix Rehabilitation & Nursing Center on April 16th, 2018, from an Acute Care Hospital – Brooklyn Hospital. She was admitted with the primary diagnoses of spinal stenosis, lumbar region without neurogenic claudication. She had difficulty in walking and muscle weakness and needed rehabilitation and recovery.

When Ms. Clyde was admitted, she was greeted by the concierge at The Phoenix and was provided a welcome gift. She was very quiet and passive at first, but as she stayed longer, she was very happy with her new roommate and quickly became friends. Our concierge would often stop by from time to time to assist with any inquires, in part to ensuring that she was receiving the highest quality of care and customer service, a staple of The Phoenix community.

Ms. Clyde was provided physical therapy as well as occupational therapy during her time here at The Phoenix. During the first weeks of her stay, her goals was set, and she was to work on the following: static standing, bed mobility, transfers, ambulation in distances, functional mobility during ADLs (activities of daily living), upper body dressing, lower body dressing, toileting, hygiene/grooming, commode transferring, and working on obtaining strength to be able to improve her lifestyle.

As the weeks progressed, Ms. Clyde was able to improve from total dependence with attempts to initiate, to being able to complete a task a bit better with help. Most importantly, she was able to have a fair level of static standing. Along with maintaining standing balance for one to two minutes without upper extremity support, she was able to ambulate 60 feet from initially not being able to walk.

During the final week of her stay, she was able to maintain standing balances without support against minimum resistances, and also, she was able to walk 150 feet with contact guard assist. She has achieved HPL (highest practical level) in therapy!

Ms. Clyde was discharged on June 4th back to her home. Thanks to our entire team, she was given all the necessary tools and recommendations to support a better lifestyle at home. We’ve all enjoyed your stay Ms.Clyde, and wish you the best!

Mrs. Ramos was admitted to The Phoenix Rehabilitation & Nursing Center on May 10th, 2018, from an Acute Care Hospital for rehabilitation after surgery on her right leg, which involved stitches. The diagnosis includes presence of right artificial knee joint and anemia, which often causes difficulty in walking.

As soon as she was admitted she was greeted by the concierge team and was presented a welcome gift. Followed by the concierge evaluations, each member from the following departments; nursing, dietary, food service, social work, recreation, as well as rehabilitation came to welcome her and gave an initial evaluation. The team assured Mrs Ramos that a personalized plan of care would be created on her behalf to ensure a signature healing program that would provide her with the greatest opportunity for recovery.

During the next day, Mrs. Ramos began her physical therapy session. She was able to walk the distance of 50 feet baseline from the time of her first session. In addition to this accomplishment, Mrs. Ramos also enjoyed her time spent during occupational therapy.

In just a few days of being on therapy, she was able to safely perform bed mobility tasks with modified independence without usage of side rails and even had reduced risk for falls! Mrs. Ramos also was invited to participate in recreational programming during Nursing Home Week. She often attended musical concerts during her stay here.

Towards the conclusion of her being in therapy, she was able to walk a distance of 300 feet without great difficulty. Mrs. Ramos was discharged on 5/18/2018, where she had achieved the highest level of rehabilitation. She can now walk unlimited distances as she is back in the community. Great job, Mrs. Ramos on your full recovery! We are so proud of you!

Ms. Bowen was admitted to The Phoenix Rehabilitation & Nursing Center on March 20th, from Acute Care Hospital, located in JFK Medical Center, for rehabilitation after diagnosis with anemia. Ms. Bowen also had muscle weakness and difficulty in walking. She did not have bed mobility when she arrived to The Phoenix.

Ms. Bowen started her rehabilitation with a baseline in walking the distance of 10 feet in the first week. Along with physical therapy, she also had occupational therapy, which helped her with activities in daily life. She also met with the concierge and the recreational team who was there to provide the highest quality services for her. Ms. Bowen said the place was “very nice” and was very appreciative of the all the support she got.

During the second to last week, she was able to improve her walking distance increased from 10 feet to 75 feet in CGA surface level. While she was here, the nursing staff and the recreational team treated her very well. The concierge would also check on her from time to time and bring her gifts, such as mugs and notepads which made her very happy. She was encouraged to get better and received a lot of support.

She did not have any issue with bed mobility by the time she was ready to leave during the few last weeks. Ms. Bowen was discharged on April 19th to her home. She had achieved the highest practical level from her therapy and the distance she was able to walk was 90 feet SBA. It was a very impressive achievement, from 10 to 75 to 90, and we are very proud of her. Congratulation and best of luck, Ms. Bowen!

Mr. Goodman was admitted to The Phoenix Rehabilitation & Nursing On February 6th 2018 from an Acute Care Hospital for rehabilitation after having complications walking. The diagnosis was acute Osteomyelitis located in his right ankle and foot.

Mr. Goodman’s first week at the Phoenix was not the easiest. Adapting to the new lifestyle changes is not the simplest venture. Being introduced to the world of Rehabilitation can be overwhelming, especially when the patient has a normal routine they enjoy.

During the first 48 hours, Mr. Goodman was very frustrated. The resident wanted to start walking again and leave as soon as possible. However, things took for a turn when he was met by the new concierge, the social worker, recreation staff and the nursing that was going to take care of him. It was at this time that Mr. Goodman knew that he was in good hands, and to trust the process.

Mr. Goodman’s attended physical therapy with the rehabilitation team, and had conversations with the concierge and the recreational staff. He began to understand the methods of rehabilitation, and his time at the facility became a great experience. The recreation staff and Mr. Goodman got along well; between making jokes and sharing funny, personal stories, they connected as friends.

Meanwhile the nursing staff provided him with incredible levels of quality care. The rehabilitation team also made sure he progressed from walking with assistance, to using the bicycle machine on his own, and then finally putting him on a “walker-scooter”.

By the time Mr. Goodman was ready to leave, he did not need a wheelchair, as he was using a scooter to ambulate. During the last few days, Mr. Goodman was introduced to Urban Zen where he could learn to relax and focus energy going forth. Mr. Goodman also participated in the Karaoke Talent Show hosted by our Recreation Team!

On March 21st, 2018, Mr. Goodman was discharged back home. He is now able to walk and get around in his flashy scooter in style. His journey started out as many have, restless and unsure, but after time and dedication, Mr. Goodman achieved his goals and healed!

On Tuesday, August 8th, The CareRite Centers Network announced Gary Lou as this year’s recipient of the National Concierge of the Year Award. Gary serves as the Director of Concierge Services here at The Phoenix Rehabilitation and Nursing Center.

Ms. Lipshie was admitted to The Phoenix Rehabilitation & Nursing Center on June 23rd, 2017 from New York Methodist Hospital for rehabilitation due to muscle weakness in her legs and shortness of breath.

At the hospital, Ms. Lipshie met with our Director of Marketing, who introduced all the services / amenities available to her at The Phoenix, while also sharing the community’s outstanding reputation for clinical and rehabilitative care. Ms. Lipshie showed great interest in our beauty parlor services. When she arrived, the concierge department ensure this need would be met by arranging her appointments. She was blown away by the level of customer service at The Phoenix as an appointment was made even before her arrival to the center.

The rehabilitation team arranged for physical and occupational therapy five days per week. Ms. Lipshie shared, “I know rehab is not easy because I always feel dizzy, but I am going to try my best.” After several follow ups by the team, she shared wonderful compliments on behalf of the interdisciplinary team.
“Your therapy department is really amazing, as you know sometimes I feel dizzy and nauseous, but the therapist always give me an orange juice and give me a light massage on my back and shoulder before we start our session. This made me very comfortable.”

During the first two weeks, she regained her strength from practicing on parallel bars. Currently, she is working her endurance to the point of upgrading to exercising on the stairs to practice her steps. She also attended occupational therapy to improve ADL self-care, bed mobility, and functional mobility.

On July 17th, 2017, Ms. Lipshie was discharged home with a walker! She’s able to now walk short distances and almost two flights of stairs! Congratulations on your progress!

With this being the 2nd time with our community she was already very familiar about the services, amenities, nutrition and care to be expected; we were all very excited to begin this journey together! When Ms. Davis arrived, she was welcomed by the interdisciplinary team and introduced to what would be expected over the new few days by the concierge department. She was introduced to her nurse manager, social worker, dietitian and therapists; some of which she even remembered from her prior visit! Ms. Davis was scheduled to have the same physical and occupational therapists as last time, who assisted her greatly in making such incredible progress.

The concierge department arranged for Ms. Davis to also have beautician services every week through the recreational team. Hair appointments and manicures were arranged to ensure that she would feel as comfortable as possible, ultimately supporting her health and overall well-being while at The Phoenix. She shared, “you guys really pay attention to what I personally like and what I dislike.”

The team also scheduled Ms. Davis to receive physical and occupational therapy five days a week. Her goal was to be able to walk up two flights of stairs and to also be able to walk with a cane without any obstacles. During the first two weeks, she regained her strength from practicing on parallel bars, later graduating to the stair trainer for additional support. She also attended occupational therapy to improve her activities of daily living, practice self-care, bed mobility, and functional mobility.

On June 12th, 2017, Ms. Davis was discharged home with simply a cane and she is now able to walk short distances and stairs. Congratulations for all of your incredible progress, Ms. Davis!

Ms. Chalk was admitted to The Phoenix Rehabilitation & Nursing Center on May 12, 2017 from Brooklyn Hospital Center for rehabilitation after a knee replacement procedure.

When Ms. Chalk first arrived, she shared deep dismay with her need for admitting to an in-patient rehabilitation center and the inability to immediately return home. She went on to share a little about herself, describing her next step to move to Texas with her family. Ms. Chalk’s physician recommended for her to undergo a knee replacement prior to moving such a long distance. Once extremely active, independent and mobile, she now felt helpless.

Ms. Chalk was introduced to the wide-array of services and amenities offered at The Phoenix, calming her peace of mind and reassuring her that she would receive unprecedented levels of care through a patient-centered and holistic approach. She was also introduced to the members of the team overseeing her care including that of her nurse manager, dietitian, social worker and her rehabilitation therapist. Ms. Chalk was reaffirmed that there were multiple professionals at The Phoenix who would assist her in her journey of healing and recovery.

With the understanding that her intent was to return to independence as soon as possible so that she could move to Texas, we scheduled her physical and occupational therapy five days a week to support a vast and quick recovery. Her goal was to achieve independent functionality in two weeks. The journey of recovery was presented as an all-encompassed regiment that would take a great deal of endurance and will to fulfil. She worked with her therapist very closely with the incredible improvement of walking with the aid of a walker in just four days!

As a diabetic, Ms. Chalk was also supported with a healthy and nutritious therapeutic diet. She shared that she loved the selection provided at The Phoenix. Once she began to feel stronger, she started to participate in most of the activities provided by the Recreation Department. With the help and support from the entire interdisciplinary team, Ms. Chalk was discharged home with a walker on May 25th, 2017! She was so happy and thrilled with the level of care and compassion she received while at The Phoenix. We wish you nothing but the best as your new journey begins in Texas, Ms. Chalk!
