Mr. S was admitted to The Phoenix Rehabilitation and Nursing Center on April 21, 2025, from Brooklyn Hospital Center and was initially admitted for an unspecified fall and a previous car accident, causing him to be less mobile.
During his first 24 hours at The Phoenix, Mr. S shared that his transition had gone smoothly and that the staff had been incredibly welcoming. He was warmly greeted by our multidisciplinary team, who took the time to introduce themselves and provide a thorough overview of what he could expect in the coming days. The team clearly outlined the services and support available to him, helping him feel informed and comfortable in his new environment. Mr. S also noted that the concierge team was especially helpful. As this was his first experience in a rehabilitation and nursing center, he had many questions. The team reassured him that they were available to assist with any questions or concerns, which provided him with a great deal of comfort. Overall, his experience was positive, and he felt well-supported throughout his introduction to The Phoenix.
During his first week with us, Mr. S remained highly engaged in his therapy and demonstrated a strong determination to make progress. Mr. S shared that he was previously a tour guide in New York City and was accustomed to walking extensive distances. As a result, he was eager to begin moving again as soon as possible, expressing how much he missed being active. Despite facing significant challenges, he consistently gave his best effort each day. Staff often observed him taking extra steps around the dining area or out on the patio, demonstrating his determination and commitment to his recovery.
At the beginning of his stay, Mr. S required assistance with essential activities such as transferring from the bed, walking, and performing daily tasks. Our dedicated nursing team provided around-the-clock support, ensuring his safety and stability seven days a week. Concurrently, our therapy team played a crucial role in his rehabilitation process.
Upon admission, Mr. S presented with significant muscle weakness, difficulty maintaining balance, and challenges bearing weight on his legs, particularly while walking or navigating stairs. These limitations stemmed from a recent joint replacement surgery, which left him unable to ambulate more than a few steps or cover a distance of 100 feet on a flat surface. He required assistance for even short walks and for standing up safely. Despite these challenges, Mr. S maintained a consistently positive attitude and demonstrated a strong commitment to regaining his independence. Our rehabilitation team provided therapy sessions daily, five days a week, focusing on improving his strength, mobility, balance, and overall ability to perform activities of daily living.
Thanks to his determination and the high-quality, personalized care provided by our multidisciplinary team, Mr. S made remarkable progress in a relatively short time. With daily therapy, personalized nursing support, and continuous encouragement, he achieved substantial gains in strength, mobility, and independence.
By his final week at The Phoenix, Mr. S expressed great excitement about returning home and resuming an active lifestyle. A former tour guide, he was accustomed to walking several miles each day, and he looked forward to regaining that level of activity. At discharge, he was able to ambulate over 200 feet with supervisory assistance using a cane, transfer from bed to chair with only supervision, and independently perform his daily self-care routines.
