How did this patient hear about The Phoenix Rehabilitation and Nursing Center? The patient was a former resident of the community in 2020
Details of Experience:
Ms. Baker was admitted to the Phoenix Rehabilitation and Nursing Center on April 9th, 2021, from Brookdale Hospital, where she was initially admitted for a hip replacement. During her 24hrs in the community, Ms. Baker was welcomed by our nursing team, as well as our Concierge. Her personal Social Worker also greeted her and offered emotional support during her stay.
During her first week in the community, Ms. Baker began physical and occupational therapy. Her goals were to walk without pain and continue her daily activities once she went home independently. At first, Ms. Baker became very depressed because she didn’t see progress. However, our recreation and the concierge team kept her motivated. They provided emotional support with daily activities to keep her distracted and have her in a positive mindset to be motivated for therapy. As the days went by, she became a happier and energetic person. She went from being in bed when the therapist would walk in the room to being up and ready with makeup and flashy jewelry, waiting for her therapist to start her day.
Throughout Ms. Bakers’ last week at the community, she progressed immensely. Prince, the Physical therapist, became her favorite. He helped her become stronger with her legs and more confident in her walking abilities. She stated, “she felt like a new person and was ready to return home.” She could walk independently and get dressed on her own with no assistance, which was her primary goal.
Ms. Baker left with a positive view on rehabilitation. She stated she was very grateful for our excellent rehab team.
