Ms. Johnson’s journey at The Phoenix began on December 12, 2019, arriving from The Brooklyn Hospital for Rehabilitation as needed for her wound care left femoral neck. She was very pleased with her transition to the community.
Over the course of her first 24 hours, Ms. Johnson’s perception of the community was very positive, she greatly enjoyed Physical Therapy and quickly became one with our team from the very start. Our rehabilitation team provided her tools, resources, and guidance to support her standing, balance, posture, and movement.
She often complimented how clean our community was and how friendly our environmental services professionals were day-to-day. Ms. Johnson had positive, great things to share about all departments and how their commitment to patient care was very noticeable through their daily practices.
Over the course of her first week, Ms. Johnson’s positive energy made all the difference in her healing. She was made comfortable by her requested accommodations and found great appreciation in the concierge services department. She made incredible progress and was slowly beginning to understand her limitations, but even greater, her potential. Our specialty wound care team guided her through education on how best to care for her wounds and what precautions to be wary of when she returned home.
Our therapy team worked alongside her to forecast home-like simulations of adaptability and movement to ensure that she would not only be returning home but successful in doing so as she shared that she has always been quite independent.
On January 17th, she received incredible and coveted news that she would be able to discharge home as she exceeded all set expectations and goals for herself. Our dedicated social services team partnered with our clinical team to establish light home care services for her upon returning home.
Ms. Johnson was discharged on January 20, 2020, to her home with support from her daughter. She will be receiving short term home care services to supplement the incredible healing and progress she has made here at The Phoenix. We wish Ms. Johnson continued healing and good fortune as she returns to the powerful woman she once was prior to her hospitalization.
