Patients Age: 53
Admission Date: 8/27/15
Admitted From: Brooklyn Hospital
Discharge Date: 10/5/15
Discharged To: Home
Length of Stay: 40 Days

Ms. Aultman was admitted to The Phoenix Rehabilitation & Nursing Center after her stay in Brooklyn Hospital because she has to be on antibiotics for 41 days as well as she has joint problems. Upon her admission, I went to greet her with welcome gift. Also at the same time, educated her with what our facility offers. She’s in one of the renovated rooms, but the brand new TV was broken and also missing a nightstand. Had to call Maintenance to help our right away.

Ms. Aultman was a person with a lot of positive attitudes. She knows that she’s here 41 days as well as rehab also. She attended PT and OT 5 days a week and she did very well.
She said : “I am going to do whatever the therapy tells me to do, I want to get better, feel stronger, so I can be independent when I leave here”

She ate in our main dining room everyday and she attended all the activities.

Upon Discharge, she said: “I want to thank everyone here, in the future if needed, definitely I’ll come back here for rehab without a doubt”
